Documents to be submitted
- Paying Patient :
Nil for walk-in direct patients, Any id proof available for name verification.
- Doctor's Admission Note :
Whenever admission advised by any doctor.
- Medical Claim :
Medical claim Card & current + previous year Insurance policy paper + Photo identity proof e.g. aadhar, pan card.
- For Security Purpose :
BRING Blank cheque at time of admission. Which will not be deposited & is returnable. Cheque required so that procedure is not held up, till the time authorization process is over.
- Corporate :
In case of corporate patient Corporate credit authority letter .

- Admission Procedure:
An admission application form + Rules & regulation information (available at counter Procedure to be filled by the patient and/or nearest relative of the patient, mentioning correct details of patient.
- Deposits :
Please note: This deposit is non-refundable if cancellation of booked OT slot by patient.
- Bed Allocation :
The institute shall allot a bed to a patient as per choice depending upon the availability of beds. In case the bed of choice is not available, arrangements shall be made as per mutual convenience.
- Visitor's Pass :
Attendentant & visitor (valid in visiting hours only) pass shall be issued per patien in Day Care, Lower median, Higher Median, First and Deluxe class.
Other Specialities
Patient Transfer
Once the patient opts for a particular class (even in I.C.C.U.), institute does not allow transfer to lower class than the existing one.
- From Lower to Higher category bed :
Patient shall be charged as per the rates of the higher class, with retrospective effect from the date of admission in the institute.
- From Ward to MICU/SICU :
The patient shall be charged as per the class of bed occupied immediately prior to transfer.
- From Ward to I.C.U/O.T./Cath Lab :
Patient vacates the bed occupied prior to transfer. However if the patient’s relative intent to occupy the room it can be arranged on availability of the room, and shall be separately charged at full rates. Prior permission of hospital administrator is mandatory for this arrangement.
Billing and Payment
- For the purpose of indoor billing, admission & discharge cut off time is 10.00 AM to 10:00 A.M (Next Day). Patients overstaying package days would be charged extra: general patient as per days stay From 10:00 A.M. to 4:00 P.M. Extra Half day charge Beyond 4:00 P.M. - Full day extra charge
- All services are charged and received in Indian Rupees. Payments are accepted in Cash/Demand Draft / Pay Order/ RTGS/ NEFT
- For any interventional/surgical procedures undertaken AFTER 7:00 P.M. or on Sunday/hospital holidays, patient will be charged additional emergency charges @ 25% of regular tariff of operation theatre and doctor's professional fees.
- In case of patient discharge after 6:00 P.M., final bill shall be prepared on the next working day and discharged patients are requested to pay an approximate amount in advance at the cash counter.
- Interim bill memo will be given to on daily bases or enquiry has to be made at admission counter for current status of bill.
- SMS will be sent on daily bases around 8:30 PM to register Mobile Nos as mentioned on the admission form. In case current bill amount is greater than deposit amount . Interim bill (draft bill) will available at admission Counter bases.
Discharge and Refund
- Regularly patients are discharged between 8:30 A.M and 6:00 P.M.
- In case of an unplanned discharge after 6:00 P.M., the final bill shall be prepared after 09:00 A.M. next morning and such discharged is to be covered by a security deposit as found necessary by the management.
- Billing and Discharge formalities may consume up to 2 hrs. after consultant's discharge advise. + Further Sanction for TPA which is sent at time of discharge.
- Original payment receipts are required to effect any monetary transaction & Final billing (i.e. Time of Discharge)
- For cash Refund of Final Billing (Timing 8.00 A.M to 11.30 A.M) or for Final bill requirement call Billing Department Extn.: 253/236.
- All refunds are paid by cheque only, after a period of 1 week from the date of discharge between (Timing 10:00 A.M. to 4:00 P.M.) (Contact Accounts Department Extn.: 256 /235)
- For Retention Deposit collection. Kindly call Marketing Department extension 301/ 302 /225 after 3 Months. Retention Deposit will be refunded only after payment is received from INSURANCE/TPA. Refund will handed over to Authorized person. Kindly bring original Retention payment deposit slip + ID Proof while coming to collect refund.
- Retention collection SMS will be sent on mobile number given to marketing department or one can call department personally after 3 Months for retention amount.
- Kindly keep the Retention Deposit Slip properly & safely. Required at The Time of collection retention Deposit.
- Retention Deposit Refund Timing 10.00 A.M to 01.00 P.M.
- No Refund on Sundays / Hospital Holidays
Report Collection
- All reports of the O.P.D. can be collected from the respective department, within 1 week from the date of getting the investigation done.
- Any I.P.D investigation reports / x-ray films not given at the time of discharge , Should be collected from the respective ward / departments. Within 1 week
Certificates/Breakups/Estimates
- For any break-up or patient record Xerox required . Contact Medical Record , break-up will be available after after 3 working days of application given to hospital on chargeable bases .

- ICU Timing :
Children below 10 years not allowed
Pharmacy
24 hrs. hospital pharmacy supplies all medicines and consumables to admitted patients as well as outside patients.
Cafeteria
The institute serves only vegetarian food. Admitted patients are served all meals and beverages prepared under expert supervision and guidance of in-house dietician.
Cafeteria services are available for patients, relatives and attendants from 7:00 A.M. to 9:00 P.M.
Outside food products are not allowed within the institute and cafeteria.
Valuables
Please Do Not
- Take kids below age of 10 years.
- Take flowers.
- Use mobile phones. (It interferes with patient monitoring)
- Take outside food.
- Smoke
- Consume Alcohol within the institute.
- On basis of Availability of bed, Patient can be admitted in the institute round the clock.

- Mediclaim: Mediclaim Card & current + previous year Insurance policy paper + Photo identity proof for example aadhar card, pan card, Blank cheque. In Case Of Corporate Patient Corporate Credit Authority Letter. Contact Marketing Department Extn.: 225/301/302 (Timing 10.00 A.M To 4.00 P.M) On Weekdays.
- Admission Procedure: An admission application form + Patients rights & responsibilities information (available at registration counter. admission Application Form to be filled by the patient and/or nearest relative, mentioning correct details of patient.
- Deposits: Payment through Cash (upto Rs 1.99L) /Demand Draft/RTGS/NEFT/UPI/Card to be made as per existing schedules for getting admission in the Institute.* Demand Draft to be in name of “Sir Kikabhai Premchand Trust Settlement No. XI “. For reservation of an operation theatre slot Rs. 5,000/- (Orthopaedic) & Rs. 10,000 (Cardiac) to be deposited along with surgeon’s note to that effect. Please note: This deposit is non-refundable on cancellation of booked OT slot by patient.

- If patient chooses to upgrade the class, patient shall be charged as per the rates of the higher class with retrospective effect from the date of admission.
- Patient vacates the bed occupied prior to transfer. However if the patient’s relative intent to occupy the room it can be arranged on availability of the room, and shall be separately charged at full rates. Prior permission of hospital administrator is mandatory for this arrangement.

- SMS will be sent on daily basis around 8:30 PM to registered Mobile No as mentioned on the admission form, In case current bill amount is greater than deposit amount. Interim bill (draft bill) will be available at admission counter.
- Patients are discharged between 8:30 A.M to 6:00 P.M.
- In case of an unplanned discharge after 6:00 P.M., the final bill shall be prepared after 09:00 A.M. next morning and such discharged is to be covered by a security deposit as found necessary by the management.
- Turn around time for discharge process- Paying patients: 3-4 hrs, TPA patients: 7-8 hrs.
- All refunds (Above Rs. 10,000/-) are paid by cheque only, after a period of 1 week from the date of discharge between (Timing 10:00 A.M. to 4:00 P.M.).
- No Refund on Sundays / Hospital Holidays
- All reports of the O.P.D. can be collected from the respective department, within one week from the date of getting the investigations done
- Any I.P.D reports / x-ray films not collected at the time of discharge should be collected from the respective wards / departments within one week from the date of discharge.
- Copies of indoor case papers will be emailed in PDF format to the patient/authorised person within 3 days from the date of submission of approved application to the Medical Record department on chargeable basis
- Visiting hours from 4.00pm to 6.00pm.
- Children below 12 years are not permitted to visit patients.
- Patients can be admitted round the clock provided beds are available. All admission patients should undergo Registration process.
- Hospital Pharmacy services are available round the clock.
- Cafeteria Services Are Available For Patients, Relatives And Attendants From 7:00 A.M. To 9:00 P.M.
- Outside Food Is Not Allowed.
